What's on the Web: Financial, Personnel Forms Updated

The financial and personnel forms that desk administrators, department heads and other administrative staff need are posted on the college's Web site for easy access. Several of the forms were updated late last year, and staff members who store copies on their computers are encouraged to save the new versions over existing ones.
The forms are located at http://coehp.uark.edu/admin_resources.php. (The page gives instructions if the staff member needs the password.)
According to Brandi Maples, director of financial affairs for the college, the Purchase Request and the Travel Request were revised. Most changes are minor but certain details do need to be obtained for processing. The Extra Compensation form that was originally on the Web site still included Social Security Number information on it so that has been removed and it now asks for employee ID instead. This form will now be interactive as well.
The forms that had some significant changes were the Request for Summer Pay or Extra Compensation Form, which was formerly called "Request for Summer Research," and the Personnel Form, which was formerly called the "New Employee Request."
"We felt the need to update the Summer Research form because the assumption was that it was only to be used for summer research, and now the form includes information on summer teaching as well as extra comp information," Maples said. "The New Employee Request was being used for more than just new employees, and we also needed more information on GA's. This should help us to gather all of the information that we need and cut down on calls for clarification."
