In order to continue to meet the criteria of various College of Education and Health Professions scholarships and minimize the time constraints placed on faculty members who have been asked to write recommendations for eligible students, the Dean's Office has prepared a common form to be used by faculty who are making recommendations for students. Faculty are not obligated to use this form. This is only an option for you to use if you so choose. Formal letters of recommendations will continue to be accepted.
Students have been contacted regarding a request for additional materials for their scholarship application. We wanted to let you know that students will be most likely be asking for letters of recommendation and this form can be used instead. The deadline for receipt of completed forms is Thursday, March 7, 2013. Please submit forms/letters to Lori Foster via e-mail at firstname.lastname@example.org. Please resave the document to reflect both the student's name and recommender's name. If you would rather deliver a hard copy, please send it to 324 Graduate Education Building.
